Academic Dismissal Appeals Procedure
Undergraduate students who have been dismissed from the College of Health and Human Sciences may appeal that decision by submitting a formal letter of appeal to Dean Derryl Block.
Appeal letters should include details relative to the following factors:
- information about circumstances that lead to academic difficulty
- actions to be taken by the appellant to change academic performance if allowed to continue
- any other relevant factors
Your appeal letter should also include the following information:
- Z-ID
- Telephone number
- Return address
- Student signature
Appeal letters may be submitted by fax at (815) 753-6169 or by mail to Dean Block (email appeals will not be accepted). Letters should be sent as soon as possible. Appeals will be reviewed in the order received with priority given to letters received by 4:30 p.m. on Friday, August 2, 2013.
Students will be notified, in writing, of the Dean's decision.
