Academic Dismissal Appeals Procedure

Undergraduate students who have been dismissed from the College of Health and Human Sciences may appeal that decision by submitting a typed formal letter of appeal to Associate Dean Beverly Henry.

Appeal letters should include details relative to the following factors:

  1. information about circumstances that lead to academic difficulty
  2. actions to be taken by the student to change academic performance if allowed to continue
  3. any other relevant factors

Your appeal letter should also include the following information:

  • Z-ID
  • Telephone number
  • Return address
  • Student signature

Appeal letters may be submitted by fax at (815) 753-6169, by mail to Associate Dean Henry, or in person (email appeals will not be accepted). Letters should be sent as soon as possible. Summer Appeals will be reviewed in the order received with priority given to letters received by 4:30 p.m. on Wednesday, August 17th, 2016.  

Students will be notified, in writing, of the Associate Dean's decision.
